Roles & Responsibilities
- Provide compassionate, hands-on patient care and support daily needs
- Administer medications and treatments safely and accurately
- Monitor and document vital signs, patient status, and care activities
- Collaborate with nurses and interdisciplinary team to implement care plans
- Communicate clearly with patients and families to educate and reassure
- Ensure accurate charting and compliance with facility policies and standards
Qualifications
- Valid Licensed Practical Nurse (LPN) license for New Hampshire, or compact state license as applicable
- Current Basic Life Support (BLS/CPR) certification or willingness to obtain
- Comfortable working 12+ hour day shifts and adaptable to per diem scheduling
- Strong clinical judgment, attention to detail, and a patient-first attitude
- Excellent communication skills and ability to work well with a team
- Previous LPN experience in a long-term care or similar setting preferred
Education
- Practical Nursing diploma or certificate from an accredited program
- High school diploma or equivalent
Knowledge & Skills
- Proficient in medication administration and basic nursing procedures
- Skilled in taking and interpreting vital signs and patient assessments
- Familiarity with electronic health records and accurate documentation
- Ability to prioritize tasks in a fast-paced clinical environment
- Strong infection control practices and patient safety focus
