Shift Type: Varies (Per Diem), 7:00 AM - 7:30 PM Day shift (12.5-hour shift, includes 30-minute meal break)

Candidate Pay Rate: $33 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to assigned residents
- Administer medications and treatments safely and accurately
- Monitor and document vital signs, patient status, and changes in condition
- Collaborate with the care team to develop and follow individualized care plans
- Perform wound care, dressing changes, and basic clinical procedures as needed
- Communicate clearly with patients, families, and interdisciplinary staff
- Maintain accurate charting in the electronic health record and follow facility policies
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of practice (New Hampshire) or eligibility to obtain one
- Current BLS/CPR certification (or willingness to obtain)
- Comfortable with 12-hour day shifts and providing hands-on patient care
- Strong communication skills and ability to work collaboratively in a team
- Previous LPN experience in a long-term care or acute setting is a plus
Education
- Graduate of an accredited Practical Nursing program (LPN certificate or diploma)
Knowledge & Skills
- Medication administration and documentation best practices
- Basic clinical skills: wound care, injections, IV assistance, vitals
- Familiarity with electronic health record charting
- Infection control and standard precautions
- Patient assessment and reporting changes to the care team
