Roles & Responsibilities
- Provide hands-on, compassionate nursing care to residents and patients
- Administer medications and treatments safely and document accurately
- Perform routine assessments and report changes in condition to the care team
- Assist with activities of daily living to promote dignity and independence
- Collaborate with nurses, providers and support staff to coordinate care
- Maintain accurate, timely charting and follow facility policies and protocols
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Hampshire required
- Completion of an accredited Practical Nursing program
- Current BLS/CPR certification preferred or willingness to obtain
- Comfortable with medication administration, patient assessments, and documentation
- Dependable, team-oriented, and committed to patient-centered care
- Previous experience in long-term care or similar settings a plus
Education
- Graduation from an accredited Practical Nursing program
- LPN diploma or equivalent and required state licensure
Knowledge & Skills
- Strong clinical assessment and vital sign monitoring skills
- Medication administration and basic wound care knowledge
- Familiarity with infection control and patient safety practices
- Clear, compassionate communication with patients, families and teammates
- Reliable documentation in electronic medical records or paper charts
- Good time management and the ability to prioritize in a busy setting
