Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and record vital signs, reporting changes to the nursing team promptly
• Support safe mobility and fall prevention practices for all residents
• Assist with feeding, bathing, grooming and toileting while preserving dignity
• Document care accurately and communicate clearly with interdisciplinary team members
• Maintain a clean, safe environment and follow infection control procedures
• Build rapport with residents and families to enhance comfort and quality of life

Qualifications

• Current CNA certification required (Certified Nursing Assistant)
• T-CNA or facility-specific training/certification as noted by the employer
• Active, unrestricted state CNA license or ability to obtain for the assignment
• Previous experience in long-term care, skilled nursing or similar settings preferred
• Basic CPR certification preferred or willingness to obtain
• Clear background check and ability to pass required health screenings

Education

• High school diploma or equivalent required; completion of an accredited CNA training program

Knowledge & Skills

• Strong patient care skills: ADLs, transfers, basic clinical observations
• Comfortable taking and recording vital signs and reporting changes
• Familiarity with infection prevention, safety protocols, and patient handling
• Good communication and teamwork skills with clinical staff and families
• Basic charting and documentation abilities (paper or electronic records)
• Adaptability to new facilities and ability to follow established care plans
