Roles & Responsibilities

• Provide compassionate direct patient care and support residents with activities of daily living
• Administer medications and treatments accurately and document per facility policy
• Assess and monitor patient condition, reporting changes to the interdisciplinary team
• Create clear, timely charting in the electronic health record and maintain clinical documentation
• Collaborate with nurses, therapists and facility staff to implement individualized care plans
• Promote a safe, clean and comfortable environment and follow infection control standards
• Support admissions, discharges and transfers as needed while maintaining patient dignity

Qualifications

• Current LPN license or Practical Nurse credential; Maine licensure or eligibility preferred
• Current BLS/CPR certification
• Ability to pass background check and meet facility immunization requirements
• Reliable transportation required; facility notes that rideshare services may be unreliable in the area
• Strong communication skills and a team-first attitude
• Flexibility to work assigned evening schedule for the duration of the assignment

Education

• Practical Nursing diploma or equivalent required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and monitoring clinical changes
• Familiarity with standard infection control procedures
• Comfortable using electronic health records for charting
• Time management and prioritization in a fast-paced care setting
• Professional communication with patients, families and the care team
