Roles & Responsibilities

• Provide compassionate, evidence-based occupational therapy interventions under the direction of the OTR to help residents regain independence
• Implement individualized treatment plans and adapt activities to meet each resident’s functional goals
• Collaborate with nursing, rehab, and care team members to coordinate care and ensure consistent progress
• Document accurate, timely clinical notes and progress reports in the medical record
• Educate residents and families on home programs, adaptive equipment, and safety strategies to support long-term outcomes
• Monitor and report changes in resident status and participate in care plan meetings to optimize results

Qualifications

• Current COTA credential (Certified Occupational Therapy Assistant)
• Texas state licensure or eligibility to obtain Texas COTA licensure
• Current CPR/BLS certification (or willing to obtain prior to start)
• Experience in skilled nursing, long-term care, or rehab settings preferred
• Strong communication skills and ability to work collaboratively with interdisciplinary teams
• Ability to pass background and health screenings per facility policy

Education

• Associate degree from an accredited Occupational Therapy Assistant program or equivalent completion of an approved OTA curriculum

Knowledge & Skills

• Skilled in therapeutic activity design to improve ADLs and functional mobility
• Proficient with clinical documentation and outcome reporting
• Knowledge of adaptive equipment, splinting basics, and safe patient handling
• Ability to assess functional status and translate assessments into measurable goals
• Comfortable coaching family members and caregivers on carry-over strategies
