Roles & Responsibilities
- Provide compassionate, patient-centered care to residents at RiverRidge Ctr
- Administer medications and treatments accurately following care plans
- Perform timely and thorough documentation to keep the care team informed
- Monitor patient status and report changes to the interdisciplinary team
- Support activities of daily living and promote patient dignity and comfort
- Collaborate with colleagues to maintain a safe, efficient, and positive environment
Qualifications
- Licensed Practical Nurse (LPN) - active license required
- Comfortable working the 2:30 PM - 11:00 PM evening shift
- Able to pass facility credentialing and background checks
- Strong communication and teamwork skills to coordinate patient care
- Willingness to engage in ongoing learning and follow facility policies
Education
- Graduate of an accredited practical nursing program or equivalent
Knowledge & Skills
- Medication administration and basic clinical nursing skills
- Patient assessment and observation with accurate reporting
- Clear documentation skills and familiarity with care plans
- Infection control practices and safe patient handling
- Compassionate bedside manner and effective communication
