Roles & Responsibilities

• Screen, evaluate and rehabilitate patients with physical functioning disorders and wound care using evidence-based practice
• Develop and implement individualized plans of care and adjust treatments based on progress and outcomes
• Deliver therapeutic interventions including manual therapy, ambulation, therapeutic exercise, modalities and assistive device training
• Provide patient, family and caregiver education and counseling to support recovery and function
• Document all regulatory and clinically required information accurately in the electronic health record and record treatment minutes per requirements
• Prepare comprehensive discharge summaries that reflect services provided and clinical outcomes
• Collaborate and communicate effectively with supervisors and interdisciplinary team members about patient progress and barriers
• Supervise and support PTAs, rehabilitation aides and students to ensure quality care and adherence to practice standards

Qualifications

• Missouri state PT license required (Compact license accepted)
• Minimum 1 year of physical therapy experience required
• Long Term Care (LTC) or rehabilitation experience required
• Proven ability to document in Electronic Health Records and accurately record treatment minutes
• Experience with wound care, standardized assessments and evidence-based treatment planning
• Comfort supervising PTAs and supporting rehabilitation aides or students
• Adherence to APTA and state practice acts, code of conduct and professional ethics

• Skilled in patient evaluation and standardized assessments
• Rehabilitation planning and outcome evaluation
• Wound care techniques and documentation
• Manual therapy, therapeutic exercise and modalities
• Competence with assistive and supportive devices
• Strong documentation skills and familiarity with device-based documentation
• Ability to participate effectively in care conferences and utilization reviews
• Productivity management and group therapy experience
