Roles & Responsibilities

• Perform comprehensive initial and ongoing assessments (physical, psychosocial, environmental) to build strong, individualized care plans
• Initiate, re-evaluate and modify Plans of Care in collaboration with physicians, patients and caregivers
• Complete OASIS assessments and other regulatory documentation accurately and on time
• Provide skilled nursing care including chronic disease management, infusion support, wound and ostomy care, and tube feeding as needed
• Coordinate care and discharge planning with interdisciplinary team members to support safe transitions
• Supervise and collaborate with CNAs and other ancillary staff to deliver high-quality home care
• Educate patients, families and caregivers to promote independence and safe self-care at home
• Maintain infection control and safe-environment practices while protecting patient privacy and confidentiality

Qualifications

• Active Illinois nursing license (RN)
• BLS certification (required at time of submission)
• Valid driver’s license and active car insurance (required)
• Home Health experience required (strong Med-Surg candidates with some Home Health experience will be considered)
• OASIS and Start of Care experience strongly preferred
• COVID and flu vaccination status required — please include vaccination status in your profile (follow submission guidance)
• Ability to provide required documents: resume, skills checklist (within 1 year), 2 supervisory references, active certifications and license

Knowledge & Skills

• Expertise in comprehensive assessments and plan-of-care development
• OASIS completion and start-of-care workflows
• Care coordination and discharge planning with interdisciplinary teams
• Strong documentation skills in compliance with federal, state and agency regulations
• Comfort using Matrix EMR on an iPad for charting and telehealth
• Infection control practices and SAFE error-prevention habits
• Clinical skills: infusion management, wound/ostomy care, catheter care, tube feeding, diabetes and chronic disease management
• Patient and family education with clear, compassionate communication
