Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments safely and accurately, following provider orders
• Perform focused assessments, monitor vital signs, and report changes promptly
• Document assessments, interventions and patient responses clearly in the electronic chart
• Collaborate with the interdisciplinary team to develop and carry out individualized care plans
• Educate patients and families on care needs, medications and discharge instructions
• Support a positive, safe environment by following infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) credential — required
• Ability to work the specified Thursday–Saturday day shifts and commute locally (LOCAL ONLY)
• Comfortable working in a fast-paced clinical environment with strong communication skills
• Basic life support (BLS) certification or willingness to obtain if not current
• Prior experience in long-term care, med-surg, or similar settings is a plus

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Solid clinical assessment skills and the ability to recognize changes in condition
• Proficient with documentation and comfortable using electronic health records
• Strong teamwork, time management and prioritization under typical clinical pressures
• Patient education, compassionate bedside manner, and infection prevention practices
