Roles & Responsibilities

• Provide compassionate, patient-centered care as an LPN at the facility
• Administer medications and treatments accurately and document thoroughly
• Monitor and report patient status, changes, and concerns to the care team
• Collaborate with nurses, therapists, and providers to support care plans
• Assist with daily living activities and promote patient comfort and dignity
• Maintain accurate, timely clinical documentation and follow facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in New Hampshire
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Previous LPN experience in a clinical setting preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and a patient-first attitude

Education

• Completion of an accredited practical nursing program
• Active LPN license

Knowledge & Skills

• Medication administration and basic IV/PO documentation knowledge
• Skilled in vital signs, wound care basics, and patient assessments
• Familiarity with charting and electronic health records
• Ability to prioritize tasks, multitask, and work well in a team
• Good interpersonal skills for patient and family communication
