Roles & Responsibilities

• Provide compassionate direct patient care in accordance with facility policies and LPN scope of practice
• Administer medications, document treatments, and monitor patient responses with accuracy
• Collaborate with RNs, physicians, and the interdisciplinary team to support care plans
• Perform focused assessments and promptly report changes in patient condition
• Maintain clear, timely charting and ensure continuity of care during handoffs
• Support patient and family education with patience and clarity to promote recovery

Qualifications

• Valid LPN license in the state of practice (New Hampshire) or compact privileges where applicable
• Current Basic Life Support (BLS) certification required
• Vaccination and TB screening per facility policy; ability to pass background and drug screening
• Comfortable working independently and as part of a fast-paced clinical team
• Strong communication skills and a patient-first mindset

Education

• Graduate of an accredited practical/vocational nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Solid knowledge of basic nursing principles, medication administration, and wound care
• Familiarity with electronic health records and accurate clinical documentation
• Ability to perform assessments, monitor vitals, and recognize subtle changes
• Good time management, prioritization, and teamwork skills
• Professional bedside manner and patient education capability
