Roles & Responsibilities

• Provide compassionate direct patient care with dignity and respect
• Assist patients with activities of daily living including bathing, dressing and mobility
• Monitor patients and report changes in condition promptly to the nursing team
• Accurately document care, observations and any interventions in the patient record
• Maintain a safe, clean environment and adhere to infection control standards
• Collaborate with nurses and interdisciplinary staff to support individualized care plans

Qualifications

• Active LNA (Licensed Nursing Assistant) credential in-state (NH/VT) or ability to obtain
• Current CPR/BLS certification preferred or willingness to get certified
• Previous experience in long-term care or acute care is a plus
• Ability to pass background check, drug screening and meet facility onboarding requirements
• Strong interpersonal skills and a patient-centered attitude

Education

• High school diploma or GED
• Completion of an approved LNA training program

Knowledge & Skills

• Patient care fundamentals and safe transfer/mobility techniques
• Vital signs measurement and basic clinical observations
• Clear, timely documentation and communication with the care team
• Infection prevention, proper PPE use, and hand hygiene
• Empathy, patience, and the ability to work in a fast-paced healthcare setting
