Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and record vital signs accurately and communicate changes to the care team
• Support safe and timely patient transfers and mobility support
• Maintain clean, organized patient environments and assist with basic hygiene needs
• Document care thoroughly and follow facility policies to ensure continuity
• Collaborate with nurses and interdisciplinary staff to deliver excellent care

Qualifications

• Active Licensed Nursing Assistant (LNA) certification (NH) or the ability to obtain state licensure
• Current BLS/CPR certification preferred or willingness to obtain
• Up-to-date immunizations and TB screening as required by facility
• Clear background check and ability to pass facility onboarding requirements
• Comfortable with patient handling, communication, and teamwork in a fast-paced setting

Education

• High school diploma or GED and completion of an approved LNA program

Knowledge & Skills

• Patient-centered care and strong observational skills
• Ability to take and accurately document vital signs
• Skilled in assisting with ADLs: bathing, dressing, feeding, toileting
• Effective communication with patients, families, and clinical team
• Basic infection control practices and safety procedures
• Dependability, empathy, and good time management
