Roles & Responsibilities

• Provide compassionate direct patient care in accordance with facility policy
• Assist patients with activities of daily living to preserve dignity and comfort
• Monitor and report changes in patient condition to the nursing team
• Document care accurately and timely to support continuity
• Communicate clearly with patients, families, and interdisciplinary teams
• Support safe mobility and fall prevention efforts
• Maintain a clean, safe patient environment and adhere to infection control practices
• Participate in team huddles and contribute to a positive workplace culture

Qualifications

• Current Licensed Nursing Assistant (LNA) credential required
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Comfortable working within a collaborative nursing team
• Dependable, compassionate, and patient-centered approach
• Previous long-term care or acute care experience is a plus

Education

• Completion of an approved LNA program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Familiarity with resident care routines and basic clinical observations
• Strong communication and interpersonal skills
• Ability to follow care plans and document accurately
• Time management and prioritization during busy shifts
• Respect for patient privacy, dignity, and safety protocols
