Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living
• Take and document vital signs accurately and report changes to the nursing team
• Assist with safe transfers and mobility support to reduce fall risk
• Maintain a clean, safe environment and follow infection control practices
• Communicate clearly with patients, families, and interdisciplinary team members
• Document care timely and accurately in the patient record
• Support admissions, discharges, and routine resident care needs during your shift

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (New Hampshire or Vermont)
• Completion of a state-approved nursing assistant training program
• CPR/BLS certification preferred or willing to obtain
• Ability to pass background check and drug screening as required
• Comfort working overnight shifts and collaborating with the care team
• Reliable, professional, and patient-centered approach to resident care

Education

• High school diploma or equivalent; completion of an approved nursing assistant program

Knowledge & Skills

• Skilled in assisting with ADLs, transfers, and mobility support
• Accurate vital sign measurement and basic clinical observation
• Strong documentation and communication skills
• Familiarity with infection control and safety procedures
• Teamwork, time management, and comfort in a fast-paced care environment
