Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Support safe patient transfers and mobility with proper body mechanics
• Document care accurately and maintain patient confidentiality
• Collaborate with nurses and interdisciplinary team members to deliver patient-centered care
• Assist with feeding, hygiene, bedside care, and comfort measures to improve patient experience

Qualifications

• Valid Licensed Nursing Assistant (LNA) certification for VT or NH
• Current Basic Life Support (BLS) certification preferred or willingness to obtain
• Minimum clinical experience in long-term care or acute care settings is a plus
• Ability to pass background and health screenings as required by the facility
• Reliable, team-oriented, and committed to compassionate patient care

Education

• High school diploma or equivalent; LNA certification required

Knowledge & Skills

• Knowledge of basic nursing assistant duties and patient safety practices
• Comfortable with charting and following care plans
• Strong communication and interpersonal skills
• Ability to prioritize tasks in a fast-paced environment
• Familiarity with infection control and hand hygiene protocols
