Roles & Responsibilities

• Provide compassionate direct patient care, assisting with activities of daily living
• Take and document vital signs, reporting changes to the nursing team
• Assist nursing staff with safe patient transfers and mobility
• Maintain accurate, timely documentation in patient charts
• Follow infection control and safety protocols consistently
• Support a positive patient experience through respectful communication and teamwork

Qualifications

• Active LNA / Licensed Nursing Assistant certification valid for VT or NH
• Current basic life support (BLS) or CPR certification preferred
• Ability to work the 3:00 PM - 11:00 PM evening shift
• Pass background check and provide required immunizations and health screening
• Comfortable working in a fast-paced long-term care setting and collaborating with a multidisciplinary team

Education

• High school diploma or equivalent and completion of an approved LNA training program

Knowledge & Skills

• Strong patient care and bedside skills
• Accurate vital sign measurement and basic assessment
• Clear, compassionate communication with patients and families
• Good documentation habits and basic charting skills
• Knowledge of infection control, safety, and mobility assistance techniques
• Team-oriented mindset and ability to follow nursing care plans
