Roles & Responsibilities

• Provide compassionate, high-quality nursing care to patients as an LPN
• Administer medications and treatments accurately and safely
• Monitor patient condition, record observations, and escalate changes
• Collaborate with RNs, physicians, and the care team to optimize outcomes
• Perform routine procedures such as wound care, dressing changes, and vital signs
• Educate patients and families on care plans, medications, and discharge instructions
• Support infection control, safety protocols, and maintain accurate documentation

Qualifications

• Active LPN license in New Hampshire or ability to obtain state licensure promptly
• Current BLS (Basic Life Support) certification required
• Clinical experience in a long-term care/acute care setting preferred
• Strong communication skills and ability to work well within a team
• Reliable, professional, and patient-centered approach to care

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in taking and interpreting vital signs and basic assessments
• Proficient with electronic medical records and clear charting
• Wound care and basic nursing procedures
• Time management, prioritization, and teamwork in fast-paced settings
• Patient and family education with empathy and clarity
