Roles & Responsibilities

• Provide compassionate hands-on care and assistance with activities of daily living to promote resident dignity
• Measure and record vital signs, reporting changes promptly to the nursing team
• Support mobility and safe transfers, using proper body mechanics and equipment
• Document resident care accurately and timely in the chart or electronic health record
• Collaborate with nurses and interdisciplinary team members to implement individualized care plans
• Maintain a clean, safe environment and adhere to infection control standards
• Build trusting relationships with residents and families through clear, respectful communication

Qualifications

• Active LNA certification (Licensed Nursing Assistant) — appropriate to practice in VT/NH
• Current Basic Life Support (BLS) certification or ability to obtain it prior to start
• Comfortable providing personal care and assisting with ADLs
• Team player with strong communication skills and a patient-centered approach
• Previous experience in long-term care or med-surg settings is a plus

Education

• High school diploma or equivalent and completion of a state-approved Nursing Assistant program

Knowledge & Skills

• Knowledge of basic nursing principles and resident care techniques
• Skill in taking and recording vital signs accurately
• Familiarity with infection control, resident safety, and fall prevention
• Strong observation skills to recognize changes in condition and report concerns
• Comfortable using electronic charting systems or paper charting as required
• Good interpersonal skills for working with residents, families, and clinical teams
