Roles & Responsibilities

• Lead and manage daily unit operations to ensure safe, high-quality patient care
• Coordinate post-acute care plans and collaborate with interdisciplinary teams
• Supervise staff, mentor team members, and support a positive workplace culture
• Oversee documentation and regulatory compliance to maintain facility standards
• Triage clinical issues and escalate appropriately to maintain patient safety
• Communicate effectively with patients, families, and external partners to facilitate transitions of care

Qualifications

• Current, unrestricted RN license for practicing in the assignment state
• Proven experience in Post-Acute Care, as required in the job notes
• Demonstrated unit management or leadership experience
• Willingness to be on-call when required and adapt to changing shift needs
• Strong communication skills and ability to coordinate multidisciplinary care

Knowledge & Skills

• Deep knowledge of post-acute care workflows and discharge planning
• Unit-level leadership and staff coordination
• Clinical assessment and prioritization skills
• Accurate clinical documentation and regulatory awareness
• Effective patient and family communication
• Ability to coordinate with case management, therapy, and ancillary services
