Roles & Responsibilities

• Provide compassionate, patient-centered nursing care following established plans
• Administer medications and treatments safely and accurately
• Monitor and document vital signs and patient status, escalating concerns promptly
• Collaborate with RNs, physicians, and the care team to coordinate effective care
• Maintain clear, timely documentation in the patient record
• Educate patients and families about care plans, medications, and discharge instructions
• Assist with admissions, transfers, and discharges to ensure smooth transitions

Qualifications

• Current LPN license in Pennsylvania (required)
• Basic Life Support (BLS/CPR) certification (required)
• Previous LPN clinical experience preferred
• Ability to pass background check and drug screen
• Immunizations and health screenings up to facility policy
• Reliable, professional approach and ability to work assigned shifts

Education

• Practical Nursing diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and patient observation skills
• Safe medication administration and basic wound care
• Clear, compassionate communication with patients and team members
• Familiarity with electronic health records and accurate charting
• Good time management, prioritization, and teamwork in a fast-paced setting
