Roles & Responsibilities

• Provide compassionate direct patient care, assisting with activities of daily living to preserve dignity and comfort
• Assist patients with bathing, dressing, feeding, mobility and toileting needs
• Measure and record vital signs accurately and report changes to the nursing team
• Document care clearly and timely in the patient record to support continuity
• Maintain a clean, safe environment and follow infection control best practices
• Communicate effectively with patients, families and interdisciplinary staff to support quality outcomes
• Support patient transfers and safe mobility with proper body mechanics and teamwork

Qualifications

• Current LNA (Licensed Nursing Assistant) certification — NH or VT eligibility preferred
• Valid state-required background checks and immunizations as required by facility
• CPR/BLS certification preferred — ability to obtain if not currently held
• Strong interpersonal skills and a caring bedside manner
• Ability to stand, lift and assist patients per facility policies

Education

• High school diploma or equivalent
• Completion of an approved LNA/CNA training program

Knowledge & Skills

• Comfortable performing personal care and assisting with ADLs
• Ability to take and accurately record vital signs
• Basic knowledge of infection control and patient safety principles
• Clear verbal and written communication skills
• Team-oriented approach and reliability in a fast-paced care setting
• Familiarity with electronic health record documentation is a plus
