Roles & Responsibilities

• Provide compassionate direct care to residents in a skilled nursing and rehab setting
• Administer medications and treatments accurately and on schedule
• Assess and monitor residents’ conditions, reporting changes promptly
• Document resident care thoroughly and maintain clear charting
• Collaborate with nurses, therapists and support staff to deliver person-centered care
• Assist with admissions, discharges and care plan updates to ensure continuity
• Promote resident safety, infection control and a clean therapeutic environment
• Communicate effectively with families to support resident wellbeing

Qualifications

• Current LPN license in Massachusetts (must have or be able to obtain quickly)
• Current BLS/CPR certification
• Previous experience in long term care, skilled nursing or rehab preferred
• Comfort with 12-hour day shifts and ability to work collaboratively on a team
• Up-to-date immunizations and TB testing as required by the facility

Education

• Graduate of an accredited LPN program
• High school diploma or GED

Knowledge & Skills

• Strong medication administration and documentation skills
• Skilled in vital signs assessment and basic clinical observation
• Familiarity with wound care, ADL assistance and rehab-focused nursing
• Proficient with electronic health records and clear clinical charting
• Excellent communication, time management and resident-centered care mindset
