Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients within your scope as an LPN
• Administer medications and document administration accurately and timely
• Perform routine assessments and report changes in condition to the care team
• Implement care plans and collaborate with RNs and interdisciplinary staff to meet patient goals
• Maintain meticulous documentation in the electronic health record and ensure regulatory compliance
• Support patient safety by following infection control procedures and safety protocols
• Educate patients and families about medications, treatments, and health maintenance
• Contribute to a positive team environment and assist with training or orienting new staff when needed

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of Pennsylvania or compact/reciprocal eligibility
• Basic Life Support (BLS) / CPR certification required
• Experience in long-term care, skilled nursing, or acute care preferred but motivated new travelers welcome
• Up-to-date immunizations and ability to clear background and health screenings
• Strong clinical assessment, medication administration, and documentation skills
• Comfortable working in a fast-paced environment and collaborating with a diverse care team

Education

• Completed LPN/Vocational Nursing program and active state LPN license

Knowledge & Skills

• Solid knowledge of medication administration and routes
• Ability to perform routine assessments and recognize changes in patient condition
• Familiarity with wound care, infection control, and diabetic care basics
• Comfortable using electronic health records and accurate charting
• Excellent communication skills with patients, families, and interdisciplinary teams
• Time management and prioritization skills to balance multiple patient needs
