Roles & Responsibilities

• Provide compassionate, hands-on nursing care to assigned patients as an LPN
• Administer medications and treatments accurately and document per policy
• Monitor and record vital signs, report clinical changes promptly to the care team
• Assist with activities of daily living and ensure patient comfort and dignity
• Collaborate with RNs, physicians, and other clinical staff to support continuity of care
• Maintain clear, timely charting in the electronic health record and follow care plans
• Support patient and family education to promote understanding and recovery

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in New Hampshire
• Current CPR/BLS certification
• Demonstrated competency in medication administration and basic nursing procedures
• Comfortable working evening shifts and collaborating with interdisciplinary teams
• Professional communication skills and a patient-centered approach

Education

• Graduate of an accredited Practical Nursing program (LPN certificate/diploma)
• High school diploma or equivalent

Knowledge & Skills

• Solid knowledge of LPN scope of practice, medication administration, and wound care basics
• Strong charting skills and familiarity with electronic health record documentation
• Ability to prioritize, multitask, and remain calm during busy shifts
• Good teamwork, communication, and patient education skills
• Basic assessment skills and ability to escalate changes in condition appropriately
