Roles & Responsibilities

• Provide compassionate direct patient care and assist residents with activities of daily living
• Monitor patient condition, record vital signs, and promptly report changes to nursing staff
• Collaborate with nurses and interdisciplinary team members to implement individualized care plans
• Maintain accurate and timely documentation in patient records
• Support safe transfers and mobility, following proper lifting and infection control procedures
• Create a welcoming, respectful environment that promotes patient dignity and comfort

Qualifications

• Active Licensed Nursing Assistant (LNA) credential valid in Vermont or New Hampshire
• Current CPR/BLS certification preferred or willingness to obtain
• Comfortable with patient lifts, transfers, and assisting with personal care
• Ability to pass background check and drug screen as required
• Positive attitude, strong communication skills, and a team-first approach

Education

• High school diploma or equivalent; completion of an approved LNA program

Knowledge & Skills

• Familiarity with activities of daily living (ADLs) and basic patient care techniques
• Ability to take and document vital signs accurately
• Basic knowledge of infection control and safety procedures
• Comfort using electronic health record systems and following care plans
• Strong observational skills and clear communication with clinical teams
