Roles & Responsibilities

• Provide compassionate, direct patient care and support with ADLs to help residents maintain dignity and independence
• Take and record vital signs, monitor changes, and escalate concerns to nursing leadership promptly
• Assist with safe transfers, mobility support, and fall-prevention practices
• Document care accurately in the patient record and communicate handoffs clearly to the care team
• Follow individualized care plans and contribute to positive resident outcomes
• Promote a clean, safe environment and adhere to infection control standards
• Build strong relationships with residents, families, and multidisciplinary team members to enhance the care experience

Qualifications

• Active Licensed Nursing Assistant (LNA) certification
• Eligibility or licensure to work in NH (VT eligibility noted in position title)
• Minimum patient-care experience preferred (facility will value hands-on LNA experience)
• Basic life support (BLS) certification as required by facility
• Comfortable working day shifts with scheduled weekend rotations
• Professional, reliable, and team-oriented with strong communication skills

Education

• High school diploma or GED and completion of an approved LNA program

Knowledge & Skills

• Skilled in ADLs, transfers, bathing, grooming and personal care
• Proficient in taking and documenting vital signs and observations
• Knowledge of infection control and patient safety best practices
• Comfortable using electronic charting or paper documentation systems
• Strong teamwork, empathy, and clear verbal communication
