Roles & Responsibilities

• Provide compassionate, patient-centered nursing care under the LPN scope of practice
• Administer medications and document accurately in the patient chart
• Perform ongoing focused patient assessments and report changes to the care team
• Assist with activities of daily living while promoting patient dignity and independence
• Collaborate with RNs, physicians and interdisciplinary staff to support safe care transitions
• Maintain accurate, timely documentation and follow facility policies and procedures
• Support infection control practices and a safe clinical environment

Qualifications

• Current LPN license in good standing (required)
• Basic Life Support (BLS) certification or willingness to obtain
• Previous LPN clinical experience preferred; new grads considered with strong clinical training
• Ability to pass facility background checks and health screening requirements
• Strong communication skills and a team-oriented attitude

Education

• Graduate of an accredited Practical Nursing or vocational nursing program (LPN diploma/certificate)

Knowledge & Skills

• Medication administration and basic IV care knowledge appropriate for LPN scope
• Focused patient assessment and vital signs monitoring
• Charting and documentation skills, comfortable with electronic health records
• Time management and prioritization during busy 12-hour shifts
• Clear communication with patients, families and interdisciplinary teams
