Roles & Responsibilities

• Provide direct, compassionate patient care following facility policies and nursing standards
• Administer medications and treatments accurately and document in the medical record
• Monitor and report changes in patient condition to the care team promptly
• Collaborate with nurses, providers, and allied staff to deliver coordinated care
• Support patient comfort, safety, and education during daily care activities
• Maintain accurate, timely documentation and participate in shift handoffs

Qualifications

• Active LPN license in New Hampshire (or eligible for state licensure) — required
• Basic Life Support (BLS/CPR) certification — preferred or willing to obtain
• Current immunizations and health screenings per facility policy
• Previous experience in long-term care, skilled nursing, or similar setting is a plus
• Professional, reliable, and comfortable working as part of a multidisciplinary team

Education

• Graduated from an accredited practical nursing program; high school diploma or equivalent required

Knowledge & Skills

• Strong clinical assessment and basic nursing care skills
• Medication administration and documentation accuracy
• Comfortable with electronic health records and charting
• Clear communication and teamwork with patients, families, and colleagues
• Good time management and ability to prioritize patient needs
