Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term/post-acute setting
• Administer medications and treatments accurately and document care thoroughly
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Assess resident condition regularly and escalate changes to the RN or provider promptly
• Educate residents and families about care plans and health maintenance in a clear, respectful way
• Support a safe, clean environment and adhere to infection control and facility policies
• Mentor and support CNAs and other care team members during shifts

Qualifications

• Active LPN license in New Mexico or ability to obtain state licensure promptly
• Current BLS/CPR certification (required)
• Previous experience as an LPN in long-term care, rehab, or skilled nursing is preferred
• Reliable, patient-centered approach with strong communication and teamwork skills
• Ability to pass facility onboarding, background check, and health screenings

Education

• Completion of an accredited Practical Nursing program (diploma or certificate)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic assessment skills and recognizing changes in condition
• Familiarity with long-term care workflows and resident-centered care
• Strong time management and prioritization during busy 12-hour shifts
• Comfortable using electronic health records and standard charting systems
