Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term/post-acute setting
- Administer medications and treatments accurately and document care thoroughly
- Collaborate with interdisciplinary team members to create and follow individualized care plans
- Assess resident condition regularly and escalate changes to the RN or provider promptly
- Educate residents and families about care plans and health maintenance in a clear, respectful way
- Support a safe, clean environment and adhere to infection control and facility policies
- Mentor and support CNAs and other care team members during shifts
Qualifications
- Active LPN license in New Mexico or ability to obtain state licensure promptly
- Current BLS/CPR certification (required)
- Previous experience as an LPN in long-term care, rehab, or skilled nursing is preferred
- Reliable, patient-centered approach with strong communication and teamwork skills
- Ability to pass facility onboarding, background check, and health screenings
Education
- Completion of an accredited Practical Nursing program (diploma or certificate)
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation best practices
- Basic assessment skills and recognizing changes in condition
- Familiarity with long-term care workflows and resident-centered care
- Strong time management and prioritization during busy 12-hour shifts
- Comfortable using electronic health records and standard charting systems
