Roles & Responsibilities

• Provide compassionate direct patient care following facility policies and care plans
• Assist patients with activities of daily living such as bathing, dressing, feeding, and mobility
• Take and record vital signs accurately and report changes to the nursing team
• Collaborate with nurses and other caregivers to support individualized patient goals
• Maintain a safe, clean, and respectful environment for residents and visitors
• Document care provided clearly and promptly in the electronic or paper chart
• Support infection control practices and patient safety protocols

Qualifications

• Active LNA license or certification (Licensed Nursing Assistant)
• Eligibility to practice in New Hampshire and Vermont as indicated by the VT/NH designation
• Prior experience in long term care or acute care is a plus but new grads who meet licensure are welcome
• Willingness to pass a background check and meet facility onboarding requirements
• Strong communication skills and a team-first attitude

Education

• High school diploma or equivalent
• Completion of an approved nursing assistant training program and state LNA certification

Knowledge & Skills

• Knowledge of safe patient handling and body mechanics
• Basic clinical skills: taking vitals, charting, and observing changes in condition
• Patient-centered approach with empathy and good interpersonal skills
• Ability to follow care plans and accept direction from licensed nurses
• Time management and basic infection prevention practices
