Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Perform routine tasks such as bathing, dressing, feeding, and toileting with dignity
• Document care accurately and timely in the patient record
• Support safe mobility and fall prevention measures for patients
• Communicate clearly and kindly with residents, families, and the interdisciplinary team
• Assist with meal service and maintain cleanliness of resident areas

Qualifications

• Current LNA license in New Hampshire or Vermont, or eligible to obtain
• Current BLS/CPR certification preferred
• Completion of background check and required immunizations
• Comfortable working the 3:00 PM - 11:00 PM shift
• Strong compassion, attention to detail, and ability to work as part of a team
• Previous long-term care or acute care experience is a plus

Education

• Completion of a state-approved LNA/NA program
• High school diploma or GED preferred

Knowledge & Skills

• Skilled in safe patient handling and ADL assistance
• Knowledge of infection control and proper PPE use
• Basic vital signs monitoring and reporting
• Clear verbal and written communication
• Time management and ability to prioritize patient needs
• Teamwork and collaboration with nurses and allied staff
• Familiarity with electronic charting or willingness to learn
