Roles & Responsibilities

• Provide compassionate, hands-on personal care and support to residents as a Licensed Nursing Assistant
• Assist with ADLs including bathing, dressing, toileting, and mobility with dignity and respect
• Monitor and report changes in patient condition to the nursing team promptly
• Help maintain a safe, clean and comfortable environment for residents
• Document care accurately and timely according to facility policies
• Collaborate with the care team to support individualized care plans and positive outcomes

Qualifications

• Current Licensed Nursing Assistant (LNA) certification for VT or NH (as applicable to assignment)
• Active basic life support (BLS) or CPR certification preferred
• Comfortable working overnight shifts and supporting varied patient needs
• Ability to pass required background checks, drug screens and facility onboarding

Education

• High school diploma or equivalent; completion of an approved LNA program

Knowledge & Skills

• Strong interpersonal skills and a patient-first attitude
• Skilled in safe patient handling and mobility assistance
• Familiarity with charting/documentation best practices
• Ability to follow care plans and escalation procedures
• Good time management and teamwork under busy shift conditions
