Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Support safe and comfortable transfers, mobility and positioning
• Document care accurately and timely in patient charts and electronic records
• Assist with meal service and ensure dietary needs are met
• Follow infection control standards and maintain a clean, safe environment
• Collaborate with the interdisciplinary team to deliver person-centered care

Qualifications

• Current LNA (Licensed Nursing Assistant) certification — eligible for VT or NH assignments
• Comfortable working the 3:00 PM - 11:00 PM evening shift
• Current Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Ability to pass background check and meet facility immunization requirements
• Strong communication and teamwork skills; compassionate bedside manner
• Previous experience in long term care or similar setting is a plus

Education

• Completion of state-approved Nursing Assistant training program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Skilled in personal care tasks including bathing, dressing and toileting assistance
• Knowledge of basic vital signs monitoring and reporting
• Familiarity with infection prevention and resident safety practices
• Good documentation practices and comfort with electronic charting
• Ability to follow care plans and take direction from RNs and supervisors
• Strong time management and ability to prioritize resident needs
