Roles & Responsibilities

• Provide compassionate direct nursing care to residents with dignity and respect
• Administer medications and treatments safely and accurately
• Assess, document and report changes in patient condition promptly
• Support activities of daily living including bathing, dressing and mobility
• Collaborate with nurses, CNAs and interdisciplinary team to coordinate care
• Follow infection control protocols and maintain a safe environment
• Educate residents and families about care plans and changes

Qualifications

• Valid Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification preferred or willing to obtain
• Previous long term care or skilled nursing experience preferred
• Ability to pass facility background check and drug screening
• Strong communication and teamwork skills

Education

• Graduate of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care and basic treatment procedures
• Familiarity with electronic charting and chart maintenance
• Infection prevention and control procedures
• Prioritization, time management and emergency response skills
• Resident-centered care and effective family communication
