Roles & Responsibilities

• Assist patients with activities of daily living with dignity and compassion
• Provide safe, dependable bedside care including bathing, dressing, and feeding
• Monitor and report changes in patient condition to the nursing team promptly
• Support safe transfers and mobility, using proper body mechanics
• Contribute to accurate, timely documentation to keep the care team informed
• Follow infection control and safety procedures to protect patients and staff
• Work collaboratively with nurses and therapy staff to deliver person-centered care

Qualifications

• Current Licensed Nursing Assistant (LNA) certification required
• Valid state authorization to practice (NH; obtain VT if assignment requires cross-state coverage)
• Current CPR/BLS preferred or willing to obtain
• Clean background check and compliance with facility health requirements (vaccinations, TB screen)
• Professional, compassionate approach and ability to work well in a team

Education

• High school diploma or equivalent and completion of an approved LNA program

Knowledge & Skills

• Comfortable assisting with ADLs and basic patient care
• Ability to observe and report changes in patient status
• Strong communication and interpersonal skills with residents and team members
• Basic infection control and safety practices
• Attention to detail and reliable documentation habits
• Team player attitude and adaptability in a fast-paced care setting
