Roles & Responsibilities

• Assess resident conditions and provide compassionate, person-centered care
• Administer medications and treatments safely and accurately
• Perform wound care, basic procedures, and follow care plans with attention to detail
• Monitor vitals, observe for changes, and escalate concerns promptly
• Document care and communicate clearly in the clinical record and with the care team
• Collaborate with nurses, therapists, and families to support holistic resident outcomes
• Educate residents and families about care, medications, and daily routines
• Support a positive, respectful environment that promotes dignity and comfort

Qualifications

• Active LPN license in the state of practice (New Mexico) required
• Current BLS/CPR certification required
• Experience in long-term care, skilled nursing, or rehab preferred
• Strong clinical judgment, reliable attendance, and the ability to work independently on weekend shifts
• Willingness to travel and adapt to different facility systems and EMRs

Education

• Graduation from an accredited Practical Nursing program (LPN/LVN diploma or certificate)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and basic nursing procedures
• Wound care and infection control principles
• Clear communication with residents, families, and interdisciplinary teams
• Proficiency with electronic medical records or willingness to learn quickly
• Time management and prioritization during busy 12.5-hour shifts
