Roles & Responsibilities
- Provide skilled occupational therapy interventions to help patients regain independence in daily living
- Assess patient functional status and develop individualized treatment plans
- Deliver ADL and mobility training, adaptive equipment recommendations, and therapeutic activities
- Document patient progress accurately and timely in the electronic medical record
- Collaborate with nurses, therapists and other care team members to support patient-centered goals
- Educate patients and families to promote carryover of therapeutic gains at home
- Participate in discharge planning and ensure safe transitions of care
Qualifications
- Current Certified Occupational Therapy Assistant (COTA) credential
- California state licensure or eligibility to practice in California
- Comfortable working across two nearby sites as part of the weekly schedule
- Previous experience in rehabilitation, skilled nursing or long-term care preferred
- Ability to pass background checks and meet facility immunization and onboarding requirements
Education
- Associate degree from an accredited Occupational Therapy Assistant program or equivalent
Knowledge & Skills
- Strong knowledge of therapeutic techniques for ADLs, transfers and functional mobility
- Skilled in patient assessment, treatment planning and progress documentation
- Familiarity with common assistive devices and adaptive equipment
- Good clinical judgment, time management and organizational skills
- Excellent communication and teamwork with patients, families and multidisciplinary teams
- Basic proficiency with electronic medical record documentation
