Roles & Responsibilities

• Provide skilled occupational therapy interventions to help patients regain independence in daily living
• Assess patient functional status and develop individualized treatment plans
• Deliver ADL and mobility training, adaptive equipment recommendations, and therapeutic activities
• Document patient progress accurately and timely in the electronic medical record
• Collaborate with nurses, therapists and other care team members to support patient-centered goals
• Educate patients and families to promote carryover of therapeutic gains at home
• Participate in discharge planning and ensure safe transitions of care

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) credential
• California state licensure or eligibility to practice in California
• Comfortable working across two nearby sites as part of the weekly schedule
• Previous experience in rehabilitation, skilled nursing or long-term care preferred
• Ability to pass background checks and meet facility immunization and onboarding requirements

Education

• Associate degree from an accredited Occupational Therapy Assistant program or equivalent

Knowledge & Skills

• Strong knowledge of therapeutic techniques for ADLs, transfers and functional mobility
• Skilled in patient assessment, treatment planning and progress documentation
• Familiarity with common assistive devices and adaptive equipment
• Good clinical judgment, time management and organizational skills
• Excellent communication and teamwork with patients, families and multidisciplinary teams
• Basic proficiency with electronic medical record documentation
