Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs to create meaningful goals
- Develop and implement individualized treatment plans using evidence-based interventions
- Deliver hands-on therapy, therapeutic exercise, gait training, and adaptive strategies
- Document progress clearly and timely to support care continuity and outcomes
- Collaborate with nurses, physicians, occupational therapists, and case managers for coordinated care
- Educate patients and families to promote independence and safe home transitions
- Monitor outcomes and adjust care plans to maximize patient progress and satisfaction
Qualifications
- Active Physical Therapist license in Massachusetts (or compact/state licensure as applicable)
- Doctor of Physical Therapy (DPT) or equivalent degree preferred
- Basic Life Support (BLS/CPR) certification required
- Prior experience in post-acute, rehab, or long-term care settings preferred
- Strong clinical reasoning, documentation skills, and team collaboration experience
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy acceptable depending on state requirements
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, manual therapy, and gait training
- Proficient with clinical documentation and electronic health records
- Strong communication and patient education skills
- Ability to prioritize caseload, manage time, and adapt to changing patient needs
- Familiarity with fall prevention, transfers, and durable medical equipment recommendations
