Roles & Responsibilities

• Assess patients' mobility, strength, balance, and functional needs to create meaningful goals
• Develop and implement individualized treatment plans using evidence-based interventions
• Deliver hands-on therapy, therapeutic exercise, gait training, and adaptive strategies
• Document progress clearly and timely to support care continuity and outcomes
• Collaborate with nurses, physicians, occupational therapists, and case managers for coordinated care
• Educate patients and families to promote independence and safe home transitions
• Monitor outcomes and adjust care plans to maximize patient progress and satisfaction

Qualifications

• Active Physical Therapist license in Massachusetts (or compact/state licensure as applicable)
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• Basic Life Support (BLS/CPR) certification required
• Prior experience in post-acute, rehab, or long-term care settings preferred
• Strong clinical reasoning, documentation skills, and team collaboration experience

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy acceptable depending on state requirements

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, manual therapy, and gait training
• Proficient with clinical documentation and electronic health records
• Strong communication and patient education skills
• Ability to prioritize caseload, manage time, and adapt to changing patient needs
• Familiarity with fall prevention, transfers, and durable medical equipment recommendations
