Roles & Responsibilities

• Assess patients’ mobility, functional status, and rehabilitation needs using evidence-based evaluation techniques
• Develop and implement individualized treatment plans to restore movement and reduce pain
• Deliver skilled therapeutic interventions including therapeutic exercise, manual therapy, and gait training
• Document patient progress and treatment plans accurately in EPIC and other required systems
• Collaborate with interdisciplinary teams (nursing, physicians, case managers) to coordinate patient care
• Educate patients and families on safe mobility, home exercise programs, and fall prevention
• Participate in competency testing and skills checklists to maintain clinical excellence

Qualifications

• Licensed Physical Therapist in Michigan (license verification required)
• BLS certification (required)
• Comfortable using EPIC for documentation and order management
• Current immunizations and screenings per facility policy (Tdap, COVID-19, MMR, Hep B, Influenza vaccine, Varicella, PPD/IGRA, Trinity 10 Panel drug screen, fit test, color vision, physical/health statement)
• Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Strong communication skills and ability to work collaboratively with clinical teams

• Patient assessment and treatment planning

• Fall-risk assessment and safe transfer techniques
• Proficient documentation skills in EPIC
• Clinical decision-making and problem solving
