Roles & Responsibilities

• Evaluate residents and create individualized, goal-driven treatment plans
• Deliver skilled therapeutic interventions to improve mobility, strength, and function
• Closely document progress, outcomes, and discharge recommendations with clarity
• Collaborate with nursing, therapy staff, and families to ensure coordinated care
• Provide safe transfer and gait training and instruct on adaptive equipment use
• Educate patients and caregivers to support carryover and maximize independence
• Participate in interdisciplinary care planning to improve resident outcomes

Qualifications

• Licensed Physical Therapist (PT) in Ohio or ability to obtain state licensure quickly
• DPT credential preferred; PTs with entry-level degrees considered (new grads welcome)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Basic Life Support (BLS) recommended or willing to obtain
• Professional communication skills and ability to collaborate on a care team

Education

• DPT or entry-level PT degree accepted (new grads encouraged to apply)

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise, and mobility training
• Knowledge of SNF documentation standards and care planning
• Ability to safely perform transfers and fall risk assessments
• Strong clinical reasoning and ability to set measurable, resident-centered goals
• Effective time management and caseload organization in a busy setting
