Roles & Responsibilities
- Assess residents’ functional abilities and create individualized OT treatment plans
- Provide skilled interventions to improve independence with activities of daily living
- Collaborate with nurses, PTs, SLPs and family members to support comprehensive care
- Document progress thoroughly and timely to meet regulatory and facility standards
- Educate residents and caregivers on adaptive strategies, equipment and safety
- Support safe discharge planning and transitions of care with clear recommendations
Qualifications
- Valid Illinois Occupational Therapy license (or ability to obtain prior to start)
- OTR or equivalent professional certification preferred
- Comfortable working in a Skilled Nursing Facility setting; New graduates welcome
- Current BLS/CPR certification or willingness to obtain
- Strong clinical assessment and documentation skills
- Professional, team-oriented attitude and excellent communication skills
Education
- Degree from an accredited Occupational Therapy program (Bachelors, Masters, or OTD acceptable)
Knowledge & Skills
- Knowledge of ADL/IADL retraining and adaptive equipment
- Skilled in therapeutic techniques relevant to geriatric and post-acute populations
- Familiarity with documentation requirements for SNF/long-term care
- Good time management and ability to prioritize caseloads
- Strong interdisciplinary collaboration and patient-family education skills
