Roles & Responsibilities
- Assess patients’ functional abilities and develop individualized OT treatment plans
- Deliver evidence-based therapeutic interventions to improve daily living skills
- Document patient progress clearly and timely in the facility's clinical records
- Collaborate with nursing and interdisciplinary teams to support safe discharges
- Educate patients and families on home strategies to maximize independence
- Adapt treatment approaches for SNF residents, prioritizing dignity and quality of life
Qualifications
- Current Occupational Therapist license (or eligible) — PA licensure preferred
- Any experience level considered — new grads welcome and encouraged to apply
- Strong clinical assessment and treatment planning skills in a SNF/skilled setting
- Comfortable with facility documentation and electronic health record usage
- Reliable, professional, and able to float to nearby units as needed
Education
- Degree or credential in Occupational Therapy (entry-level degree/credential appropriate for licensure)
Knowledge & Skills
- Knowledge of skilled nursing facility workflows and rehab priorities
- Skilled in ADL/IADL assessment and adaptive equipment recommendations
- Good clinical judgment for discharge planning and safety assessment
- Strong communication and interdisciplinary collaboration skills
- Time management and the ability to manage a varied caseload
