Roles & Responsibilities

• Evaluate and reassess patients to develop and update individualized PTA treatment plans
• Implement therapeutic exercises, gait training, and mobility programs that restore independence
• Document patient progress and treatment notes clearly and timely in the facility record
• Collaborate with therapists, nursing staff, and family to coordinate patient-centered care
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Maintain therapy equipment and ensure a safe, clean treatment environment
• Contribute to clinical problem-solving and use best practices to improve outcomes

Qualifications

• Licensed Physical Therapist Assistant (PTA) in Illinois or ability to obtain licensure prior to start
• Graduate of an accredited Physical Therapist Assistant program (associate degree or equivalent)
• BLS/CPR certification or willingness to obtain prior to start
• Any experience level welcome, including new graduates
• Comfortable working in a skilled nursing facility (SNF) setting and collaborating with interdisciplinary teams
• Ability to pass background check and meet facility onboarding requirements

Education

• Associate degree from an accredited Physical Therapist Assistant program
• Completion of PTA clinicals as part of program requirements
• State PTA licensure (or eligible to obtain) required

Knowledge & Skills

• Strong understanding of therapeutic exercise, gait training, transfers, and mobility aids
• Knowledge of SNF patient populations including post-acute care needs
• Skilled in patient assessment, treatment planning, and progress documentation
• Effective verbal and written communication with patients, families, and staff
• Basic infection control practices and safe patient handling techniques
• Professionalism, time management, and ability to prioritize caseload
