Roles & Responsibilities
- Evaluate and reassess patients to develop and update individualized PTA treatment plans
- Implement therapeutic exercises, gait training, and mobility programs that restore independence
- Document patient progress and treatment notes clearly and timely in the facility record
- Collaborate with therapists, nursing staff, and family to coordinate patient-centered care
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Maintain therapy equipment and ensure a safe, clean treatment environment
- Contribute to clinical problem-solving and use best practices to improve outcomes
Qualifications
- Licensed Physical Therapist Assistant (PTA) in Illinois or ability to obtain licensure prior to start
- Graduate of an accredited Physical Therapist Assistant program (associate degree or equivalent)
- BLS/CPR certification or willingness to obtain prior to start
- Any experience level welcome, including new graduates
- Comfortable working in a skilled nursing facility (SNF) setting and collaborating with interdisciplinary teams
- Ability to pass background check and meet facility onboarding requirements
Education
- Associate degree from an accredited Physical Therapist Assistant program
- Completion of PTA clinicals as part of program requirements
- State PTA licensure (or eligible to obtain) required
Knowledge & Skills
- Strong understanding of therapeutic exercise, gait training, transfers, and mobility aids
- Knowledge of SNF patient populations including post-acute care needs
- Skilled in patient assessment, treatment planning, and progress documentation
- Effective verbal and written communication with patients, families, and staff
- Basic infection control practices and safe patient handling techniques
- Professionalism, time management, and ability to prioritize caseload
