Roles & Responsibilities

• Evaluate residents’ physical therapy needs and develop personalized treatment plans
• Implement evidence-based therapeutic interventions to improve mobility and function
• Provide safe transfer and gait training, focusing on independence and fall prevention
• Document treatment progress and communicate changes in condition to the interdisciplinary team
• Collaborate with nurses, OT, SLP and care staff to promote coordinated, resident-centered care
• Educate residents and families on home exercise programs and safe mobility strategies

Qualifications

• Licensed Physical Therapist (PT) in Maryland or eligible to obtain state licensure
• Doctor of Physical Therapy (DPT) preferred — new graduates are welcome
• Experience or interest in Skilled Nursing Facility (SNF) practice
• Ability to work day-shift schedules and provide reliable on-site coverage (no float)
• Strong clinical reasoning, documentation skills, and teamwork mindset

Education

• Doctor of Physical Therapy (DPT) preferred; accredited PT degree required
• New graduates accepted

Knowledge & Skills

• Skilled in mobility assessment, therapeutic exercise, and gait training
• Familiarity with SNF clinical workflows and Medicare/rehab documentation expectations
• Proficient with safe transfers, ADL training, and assistive device recommendations
• Clear communicator with residents, families, and interdisciplinary care teams
• Comfortable delivering evidence-based care and tracking measurable progress
