Roles & Responsibilities
- Deliver compassionate, hands-on physical therapy care to skilled nursing residents
- Assess mobility, strength, balance and functional needs and create individualized plans
- Implement treatment plans that improve independence with transfers, ambulation and ADLs
- Collaborate with nursing and rehab team members to coordinate resident care
- Document progress clearly and timely to support outcomes and transitions
- Educate residents and families to maximize recovery and safe return to community
Qualifications
- Licensed Physical Therapist or DPT; NH license preferred or ability to obtain before start
- DPT or equivalent entry-level physical therapy degree (new graduates welcome)
- Comfortable working in a skilled nursing facility (SNF) setting
- Current BLS/CPR certification or ability to obtain
- Available for a 13-week contract beginning 2026-01-01
- Strong communication skills and a team-first attitude
Education
- DPT or physical therapy degree (entry-level PT/DPT)
Knowledge & Skills
- Skilled in evaluating and treating geriatric conditions and mobility limitations
- Knowledge of safe transfer techniques, gait training and assistive devices
- Proficient documentation skills and familiarity with SNF workflows
- Ability to develop measurable treatment goals and track outcomes
- Comfort working with interdisciplinary teams and family education
