Roles & Responsibilities

• Provide skilled physical therapy interventions to residents in a skilled nursing facility
• Deliver safe, effective gait training, transfers, and therapeutic exercise programs
• Document resident progress and care plans clearly and on time in the facility EMR
• Collaborate with nurses, OTs, SLPs, and the interdisciplinary team to optimize outcomes
• Educate residents and families on home programs, safety, and fall prevention
• Contribute to discharge planning and continuity of care with empathy and professionalism

Qualifications

• Graduated from an accredited PTA program
• Licensed or eligible for PTA licensure in New Hampshire
• CPR/BLS certification (or willing to obtain prior to start)
• Comfortable working in a Skilled Nursing Facility setting
• Open to all experience levels — any experience accepted, including new grads
• Reliable, professional, and able to travel and adapt to new facilities

Education

• Associate Degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Strong hands-on therapy skills: transfers, gait training, therapeutic exercise
• Familiarity with SNF workflows and interdisciplinary communication
• Comfortable with documentation and basic EMR use
• Knowledge of infection control practices and resident safety
• Good time management and ability to prioritize a caseload
• Excellent interpersonal skills and patient-centered care approach
