Roles & Responsibilities
- Evaluate resident functional status and create individualized occupational therapy plans
- Develop and implement therapeutic interventions to improve daily living skills and independence
- Collaborate with interdisciplinary team members to coordinate resident-centered care
- Document assessments, treatment plans, progress notes, and discharge recommendations accurately and timely
- Educate residents and families on adaptive techniques and home safety to support successful transitions
- Participate in care conferences and contribute to quality improvement initiatives
Qualifications
- Licensed Occupational Therapist (OT) — must be able to practice in New Hampshire (licensure assistance available)
- New graduates welcome; all levels of experience considered
- Experience or comfort working in a Skilled Nursing Facility (SNF) setting preferred
- Strong documentation skills and familiarity with therapy billing and regulations
- Current BLS/CPR certification preferred or willingness to obtain
Education
- Entry-level Occupational Therapy degree (Master's or OTD preferred). New grads accepted.
Knowledge & Skills
- Familiarity with SNF rehabilitation practices, ADL retraining, and durable medical equipment recommendations
- Skilled in resident assessment, goal writing, and measurable treatment planning
- Effective communication and teamwork with nursing and therapy staff
- Ability to manage caseloads, prioritize tasks, and document thoroughly
- Patient-centered approach with strong clinical reasoning
