Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans to restore function and mobility
- Deliver skilled physical therapy interventions consistent with SNF standards
- Document patient progress accurately and timely in the medical record
- Collaborate with nurses, OT, SLP and the care team to optimize patient outcomes
- Educate patients and families on home exercise programs and safe discharge plans
- Participate in care planning and weekly interdisciplinary team meetings
Qualifications
- PT or DPT credentials listed in the job skills
- Open to any experience level, including new graduates (years of experience: any)
- Experience or comfort working in a Skilled Nursing Facility (SNF) environment
- Current state licensure or ability to obtain licensure for practice in Ohio
- Active clinical reasoning and strong documentation skills
Education
- Doctor of Physical Therapy (DPT) preferred
Knowledge & Skills
- Foundational physical therapy assessment and treatment techniques
- Skilled nursing facility (SNF) care processes and interdisciplinary collaboration
- Gait training, therapeutic exercise, balance and fall prevention strategies
- Patient and family education with emphasis on safe discharge planning
- Proficient clinical documentation and outcome tracking
